Reporting Templates
Reporting MSFHR Awards in the Common CV
MSFHR Team Planning award recipients are required to submit a final report about the Team and the use of MSFHR funds in supporting the Team's planning activities. There are two parts to the reporting process:
- A written final report must be completed as per the templates below. The submitted report must be signed by the Team Leader. Electronic signatures are acceptable and the report can be submitted via e-mail to the Infrastructure Program. If an electronic signature is not available, a signed hard copy must be mailed to the Foundation.
- In addition to the report, a hard copy of an independent financial statement issued by the finance department of the Team's host institution is required. This financial statement should detail the use of MSFHR funds for the period of the award and must be jointly signed by the Team Leader and an authorized financial officer of the host institution. As per the Conditions of Award, any unspent funds at the end of the award term must be returned to MSFHR. If applicable, a cheque for the remaining balance of MSFHR funds should be included with the financial statement.
