Reporting MSFHR Awards in the Common CV

Reporting Templates

MSFHR Team Start-up award recipients are required to submit an annual progress report about the Team and the use of MSFHR funds in supporting the Team's research activities. There are three parts to the reporting process:
  1. A written progress report must be completed as per the templates below. The submitted progress report must be signed by the Team Leader. Electronic signatures are acceptable and the report can be submitted via e-mail to the Infrastructure Program. If an electronic signature is not available, a signed hard copy must be mailed to the Foundation.
  2. Funding Cohort Templates Deadline
    Cohort 1
    Funding commenced July 1, 2006

    Cohort 2
    Funding commenced July 1, 2007
    Annual Progress Report for 2007/08 (Word Doc)

    August 15, 2008

  3. In addition to the report, a hard copy of an independent financial statement issued by the finance department of the Team's host institution is required. This financial statement should detail the use of MSFHR funds for the reporting period and must be jointly signed by the Team Leader and an authorized financial officer of the host institution.


  4. Finally, MSFHR will arrange a short face-to-face meeting with the Team Leader to discuss the Team's progress and MSFHR's contribution to supporting the Team's research activities. This meeting usually occurs in October-November.
