Evaluating the renoprotective effect of acetaminophen in children with severe falciparum malaria

Acute kidney injury (AKI) complicating critical illness is an important problem, contributing to roughly 1.7 million deaths worldwide per year. Treatment is limited to dialysis, which is costly and frequently unavailable. Preventing AKI is a critical step to reduce deaths. Acetaminophen (Tylenol) has the potential to reduce AKI caused by oxidative damage from hemoglobin (released from red blood cells) and myoglobin (released from muscle cells).

Therapeutic efficacy of a novel enteral nutrition strategy in pediatric inflammatory bowel disease

Inflammatory bowel disease (IBD) is characterized by chronic, relapsing inflammation of the gastrointestinal tract, and includes Crohn’s disease (CD) and ulcerative colitis (UC).

The gold standard induction therapy for treating active pediatric CD is “exclusive enteral nutrition” (EEN), which is a nutritionally complete liquid diet provided by tube feeding that excludes normal food intake. This nutritional strategy is superior to standard induction therapies; however, treatment must be maintained for 6-12 weeks to induce remission, and relapse rates are high after stopping EEN.

Prevention of severe bacterial infections in children by optimizing protection through vaccination

Dr. Sadarangani's  research focuses on preventing severe illness and death in children by ensuring best use of vaccines to protect against three serious infections (meningococcal, pneumococcal, pertussis) which cause blood poisoning, meningitis and whooping cough.

Vaccines have reduced these infections, but we dont know if we are usng the optimal number and timing of dses. Sadarangani's goals are to ensure optimal use of these vaccines and aid development of future vaccines.

Targeting stress granules: A novel strategy to inhibit Ewing sarcoma metastasis

Metastatic disease remains the single most powerful predictor of adverse outcomes in Ewing sarcoma (ES) and other childhood sarcomas (malignant connective tissue tumours). High risk ES appears to be characterized by uninhibited outgrowth of neoplastic clones that have acquired additional genomic or epigenomic alterations, which facilitate the spread of the cancer cells.

Unraveling the genetics of severe reactions to chemotherapy: Moving towards maximum benefit with minimal harm

Drug treatments are essential for the survival of cancer patients. Unfortunately, medications needed for treatment can also cause permanent disabling side effects, severely impacting on the quality of life of patients already suffering the devastating consequences of cancer.

Although platinum-based drugs such as cisplatin are highly effective and are the most frequently used class of cancer medications, they are also accompanied by severe side effects. In fact, up to 80% of patients treated with cisplatin lose some ability to hear and/or experience kidney injury.

Rethinking early intervention therapy with Indigenous communities and families in northern British Columbia

Early intervention therapy (EIT) programs for children with developmental challenges and disabilities have been shown to be highly beneficial for young children (0-5 years) in the wider Canadian population. However, EIT programs are consistently significantly under-utilized by Indigenous communities and families. Indigenous parents and community stakeholder perspectives on EIT are largely absent in current literature, and Indigenous communities are often not consulted on how these programs are delivered.

However, recent outbreaks of vaccine-preventable diseases such as measles have heightened concern regarding parents who are vaccine hesitant (i.e. who exhibit delayed acceptance or refusal of some or all vaccines).

Emergency department overcrowding: Determinants, measures and pediatric outcomes

Children needing emergency care are brought for help to either a general emergency department (ED) that treats patients of all ages or a pediatric unit (PED) dedicated to children. In both settings, timely access to quality care is often threatened by overcrowding. There is substantial evidence about the factors causing overcrowding in general EDs and about the association of overcrowding and poor patient outcomes in adults. In contrast, we know little about the relationships among root causes, measures of overcrowding in PEDs, and pediatric patient outcomes.

Mechanisms of cardiovascular disease in children with mental health conditions

Prescriptions of second-generation antipsychotic  (SGA) medication for children in British Columbia increased 22-fold from 1996 to 2010. These medications treat the underlying mental health issues (e.g. psychosis, depression, attention deficit/hyperactivity disorder) but often come with side-effects, including metabolic syndrome.

Relationship of Neonatal Pain and Early Brain Development of Preterm Infants on Motor Outcomes at 18 months

Between 2005 and 2009, more than 16,000 infants in British Columbia were born prematurely. Prematurely born infants are at increased risk for developing motor problems that, in many cases, significantly interfere with daily life and school performance. This degree of motor difficulty is often referred to as developmental coordination disorder, or DCD. Children with DCD struggle with many typical tasks, such as tying shoes, riding a bike, handwriting or participating in sports.

Childhood lung diseases: Infectious and inflammatory mechanisms

Lungs are for life. Unfortunately, the most frequent long-term illnesses in children and babies are respiratory system conditions. Children's lungs can be damaged in many ways: bacterial and viral infections, asthma, or faulty genes causing thick mucus to accumulate in the lungs of children with cystic fibrosis. Even the oxygen and artificial ventilation needed to sustain the lives of premature babies can cause lasting lung damage.

Immunological basis for infections in prematurely born infants

Pre-term babies, those born before week 37 weeks of gestation, are more susceptible to invasive infections than full-term babies. The smallest babies born “extremely” premature (those born before 32 weeks, or approximately 1,500 grams or less of birth weight) suffer the greatest burden of infection among all age patient age groups in BC and other developed countries in general. About one in four “extremely” pre-term babies suffers from an invasive infection, which adds up to more than 8,760 new invasive infections in North America each year.
